Surrender Request Forms

Please fill out all boxes of the section that applies to you. If you cannot answer the question, put “n/a” for not applicable or “unknown” if you do not know the answer.

If possible, we would appreciate that a photo of the dog that you wish to surrender be uploaded at the bottom of the form. This helps us to find a potential foster home for the dog.

We will respond at our earliest convenience, but please keep in mind that we are a volunteer ran rescue. Be sure to double check your phone number and email address before submitting.

Are they eating and drinking normally?

Any health concerns?

For adult dogs, what is their temperament? Are they dog friendly?

Are you willing to foster until they are adopted?

Attach link to a clear photo of the dog(s)

BEFORE PRESSING SEND:

• Have you searched for the dog’s owner?

• Have you taken the dog to the nearest vet to check for a microchip? *Required*

• Have you posted the dog on public sites so that the owner can find it? (Ex: Facebook, Nextdoor, Pawboost, paper flyers around the area found)

*We require that a 72 hour stray hold be served to allow time for the owner to come forward before committing to rescue the found dog.
